We ask that you please complete this puppy adoption application form so we can get a little more info about you and the home your new puppy will be part of.  We like to make sure that our puppies find good homes since we take such great care of them...  our conscience wouldn't allow anything else. :)

Do you have a fenced yard that will provide your new puppy a safe place to play? How tall is your fence?:


Are you aware of possible health risks associated with taking a puppy, who is not up to date with their vaccinations, to pet stores, vet clinics, parks and other public ares where un-vaccinated/sick dogs may have been?:


Are you aware that the first puppy vaccination (which we provide) is NOT the only vaccination your puppy will need? Are you opposed to continuing your puppy's vaccinations schedule after bringing hime/her home?:


It is impossible to watch an active puppy every second you are with them. Injury, toxic poisoning & choking on a foreign object are only a few of many dangers your curious puppy could be exposed to when left to wander without supervision. Crate training is not only humane, but it may save your puppy's life as well as save you thousands of dollars in emergency vet bills. Do you support the idea of crate training?:
